I certify that my child enrolled is in excellent health and may participate in the strenuous physical activities required by the sport and training of soccer for the time of year, climate and geographic location.

Spartan Soccer Academy/ABC Sales (SSA/ABCS) restricts staff from administering any kind of medication, including Epi-pens. If a player registered at camp or any Spartan Soccer Academy program requires administration, a parent or responsible adult must be present.

I agree to defend and hold Spartan Soccer Academy/ABC Sales, its servants, agents, coaches, licensors, suppliers, customers, and/or employees and contractors harmless from any and all claims for injuries sustained by my child during his or her participation in the camp.

Permission is hereby granted to Spartan Soccer Academy/ABC Sales to use pictures, video and any other type of media of the campers in any promotional materials, including the website, social media, and partners of SSA/ABCS. More details are available on the Media Release Policy section of the website.

If I am over the age of 18 and am enrolling myself as a participant, I certify that permission is granted for me to receive emergency medical treatment. If needed, I certify that there are no limits to my child’s participation except as stated in writing and included with this application.

No refund for cancellation within 30 days of the camp start date. No refund for initial, $50 deposit. $25 fee for any returned checks. $25 handling fee for any refunds.

Children who leave during the program indefinitely due to injury or illness will receive a refund with medical documentation.

A credit voucher is available in place of a refund.

Any promotional offers are only valid during specified dates. All purchases or transactions prio to or after a promotional offer can not be exchanged for retroactive benefit.

Children are enrolled on a first come, first serve basis. We cannot guarantee enrollment any later than 5 days prior to camp start date.

Should inclement weather affect the viability or safety of the program, vouchers for future programs or refunds will be considered, but not guaranteed.

Camp promotional equipment cannot be guaranteed for participants registered within 14 days of the start of camp.
